What is Strabismus?
ما هو الحول؟ أسبابه وتشخيصه
Strabismus is misalignment of the eyes — one eye points toward the target while the other deviates. Causes include congenital factors, refractive errors, muscle paralysis, and systemic disease.
الحول هو عدم التقاء العينين نحو نفس الهدف. أسبابه تشمل الحول الخلقي، وعيوب الانكسار، وشلل عضلات العين، والأمراض العامة.

Treatment Options
طرق علاج الحول
Treatment depends on the type of strabismus and may include glasses (for hyperopia-related squint), eye patching for amblyopia, prismatic lenses for diplopia, or surgical correction.
يعتمد العلاج على نوع الحول وقد يشمل النظارة الطبية، وتغطية العين للكسل البصري، والمنشور الطبي، أو التدخل الجراحي.

Surgery in Detail
جراحة الحول بالتفصيل
Strabismus surgery works on the eye muscles from the outside — the eye is never opened. The surgeon detaches and reattaches muscles with precise sutures. No laser is used. Duration: 30–90 minutes under general anesthesia.
جراحة الحول تعمل على عضلات العين من الخارج دون فتحها أبداً. لا تُجرى باللزر. المدة ٣٠–٩٠ دقيقة تحت التخدير الكلي للأطفال.

After Surgery
ماذا أتوقع بعد الجراحة؟
Redness and mild swelling are completely normal. TV, phone, computer, reading and sports are all permitted from the day after surgery. Only restriction: no swimming for 4 weeks. Eye drops for 1–3 weeks.
الاحمرار والتورم طبيعيان تماماً. يمكن ممارسة جميع الأنشطة من اليوم التالي. الاستثناء الوحيد: تجنب السباحة ٤ أسابيع. قطرة مضاد حيوي لأسبوعين.

Success Rates
نسب النجاح والمضاعفات
Short-term success (3–6 months): 85–90%. Long-term recurrence: 5–10%. Need for more than 2 surgeries: less than 3%. The need for a repeat surgery does not mean an error occurred — it is an expected biological variation.
نجاح الجراحة الأولى قصير المدى: ٨٥–٩٠٪. ارتجاع الحول على المدى البعيد: ٥–١٠٪. الحاجة لأكثر من جراحتين: أقل من ٣٪. الحاجة لجراحة أخرى لا تعني وجود خطأ.

When to Call Immediately
متى تتصل بطبيبك فوراً؟
Contact us immediately if you notice: sudden vision loss, severe pain unresponsive to painkillers, thick yellow/green discharge, fever above 38.5°C, or severe increasing eyelid swelling. Note: redness alone is normal and not a danger sign.

Amblyopia (lazy eye) affects 2–5% of children. Early diagnosis and treatment — ideally before age 7 — makes a critical difference. Here are the most important questions answered.

يصيب كسل العين ٢–٥٪ من الأطفال. التشخيص والعلاج المبكر — قبل سن السابعة — يُحدث فارقاً حاسماً. إليك أهم الأسئلة وإجاباتها.

What is amblyopia (lazy eye)?
ما هو كسل العين؟
Amblyopia means reduced visual acuity in one eye compared to the other, even after correcting all underlying causes. The problem has two components: one in the eye itself, and one in the brain, which loses its ability to process the image from the affected eye.
كسل العين هو انخفاض حدة الإبصار في إحدى العينين عن الأخرى حتى بعد علاج جميع الأسباب. المشكلة ذات شقين: شق من العين ناتج عن المشكلة الأصلية، وشق من المخ الذي يفقد قدرته على استخدام الصورة التي تصله من العين.
Does different eyeglass prescription between the two eyes mean amblyopia?
هل اختلاف مقاسات النظارة بين العينين يعني وجود كسل؟
Not necessarily. If a child needs different prescriptions for each eye but sees well with glasses in both eyes, this is called a refractive error — not amblyopia. Amblyopia is only diagnosed when one eye sees significantly worse than the other even with the correct glasses.
ليس بالضرورة. إذا كان الطفل يحتاج مقاساً مختلفاً لكل عين لكنه يرى جيداً بالنظارة بكلتا العينين، فهذا يُسمى عيب انكسار وليس كسلاً. الكسل لا يُشخَّص إلا إذا كانت حدة الإبصار بإحدى العينين أقل بشكل ملحوظ من الأخرى حتى بالنظارة الصحيحة.
What causes amblyopia?
ما هي أسباب كسل العين؟
The most common causes are: (1) Unilateral strabismus — the brain favors the straight eye and ignores the deviating one; (2) Unequal refractive errors between the two eyes (anisometropia); (3) Eye diseases in childhood such as congenital cataract, glaucoma, or corneal opacity that block the visual image during the critical period of brain development.
أشهر الأسباب: (١) الحول المرتكز في عين واحدة؛ (٢) فارق في مقاسات النظارة بين العينين؛ (٣) أمراض العين في الطفولة كالمياه البيضاء الخلقية أو الزرقاء أو سحابات القرنية التي تحول دون وصول الصورة للمخ في فترة النضج البصري.
Is there a best age to treat amblyopia?
هل يوجد عمر مثالي لعلاج كسل العين؟
Yes. The best results are in children under 7 years old. Results gradually decline with age, though treatment can sometimes be effective in older children and even adults. The younger the child, the better the response — which is why early screening matters so much.
نعم. أفضل نتائج العلاج تكون قبل سن السابعة، وتقل تدريجياً كلما زاد عمر الطفل، وإن كان العلاج قد ينجح أحياناً في سن أكبر. لذا فإن الكشف المبكر بالغ الأهمية.
How is amblyopia treated?
ما هي طرق علاج كسل العين؟
Treatment has three main approaches: (1) Patching the good eye for 4–6 hours daily, forcing the brain to use the lazy eye; (2) Atropine drops in the good eye twice weekly — an alternative to patching in mild cases; (3) Dichoptic training using 3D glasses, computers or virtual reality — used alongside patching, especially for older or non-compliant patients.
طرق العلاج ثلاثة: (١) تغطية العين السليمة ٤–٦ ساعات يومياً لإجبار المخ على استخدام العين الكسولة؛ (٢) قطرة الأتروبين في العين السليمة يومين متتاليين أسبوعياً كبديل للتغطية في الحالات البسيطة؛ (٣) التمارين الثنائية بالنظارات المجسمة والكمبيوتر والواقع الافتراضي كوسيلة مساعدة.
Does patching harm the good eye?
هل تغطية العين السليمة تضرها؟
No. Patching the good eye for up to 6 hours daily causes no harm and no reduction in its vision. The good eye is fully protected throughout treatment.
لا. تغطية العين السليمة حتى ست ساعات يومياً لا تسبب أي ضرر لها ولا تؤدي إلى أي انخفاض في الرؤية.
My child refuses to wear the patch — what can I do?
ماذا أفعل إذا رفض طفلي التغطية؟
Try these strategies: make it fun by letting the child decorate the patch or play "pirates"; link favourite activities (screen time, games) to patching hours — this also stimulates the lazy eye; reward daily compliance; for older children, explain how amblyopia could affect driving or future career choices. You can also start with fewer hours and build up gradually.
جرّب هذه الطرق: اجعل الأمر مرحاً بتلوين الغيار أو لعبة القراصنة؛ اربط الأنشطة المحببة (الشاشات، الألعاب) بوقت التغطية — وهذا أيضاً يُنشّط العين الكسولة؛ كافئ الطفل على الالتزام اليومي؛ لكبار الأطفال اشرح تأثير الكسل على قيادة السيارة أو العمل مستقبلاً. ويمكن البدء بساعات أقل والزيادة تدريجياً.
Is there a surgical treatment for amblyopia?
هل يمكن علاج كسل العين بالجراحة؟
No. There is no surgical treatment for amblyopia. The condition relates to the brain's ability to process the visual image, not to the eye's optics. Laser surgery or lens implants may eliminate the need for glasses, but they do not improve visual acuity beyond the level achievable with glasses.
لا. لا توجد جراحة لعلاج كسل العين لأن المشكلة مرتبطة بقدرة المخ على ترجمة الصورة. جراحات الليزك وزراعة العدسات قد تُغني عن النظارة لكنها لا تُحسّن حدة الإبصار عن مستواها الأصلي بالنظارة.
